To register for placements whether locum, permanent, part or full time in any capacity, you will need to send us copies of the following documents;

Proof of identity – photo i.d. Passport or UK Driving licence

Proof of National Insurance (NI card, P45 or P60, DWP or HMRC letter, full wage slip) or Unique Tax Reference (UTR).

Copy of any Professional Indemnity Insurance Certificate (If required for your role).

Contact details of two professional referees* (Required for all locum posts at registration)

GPhC registration number/NMC Pin number
