To register for placements whether locum, permanent, part or full time in any capacity, you will need to send us copies of the following documents; (ATTACH DOCUMENT COPIES TO YOUR E-MAIL BELOW WHEN COMPLETING YOUR INITIAL SIGN-UP)
Proof of identity – photo i.d. Passport or UK Driving licence
Proof of National Insurance (NI card, P45 or P60, DWP or HMRC letter, full wage slip) or Unique Tax Reference (UTR).
Copy of any Professional Indemnity Insurance Certificate (If required for your role).
Contact details of two professional referees* (Required for all locum posts at registration)
GPhC registration number/NMC Pin number (If required for your role).

How to upload your documents; Smartphone or Android – take a photograph and upload OR e-mail the photos to yourself and upload from there OR attach to an e-mail and send to us with your name and type of work that interests you in the header. Remember to attach your document copies to your e-mail and send to us on our e-mail address
